The new U.S. Health and Consent Form is now live

It’s here! We’ve launched a new U.S. Health and Consent process that replaces the need for areas to manage different health and consent forms for trips and events. There is now one streamlined form, managed in YL Connect, that guides participants through all required fields. This form will be required for summer 2026 and is available now for any trip over 72 hours.  

To get started:

  1. Learn about the new process. The Learning guide has been updated – review the health and consent form section (pages 65-69). These instructions show you what you need to do. There are also additional resources on Staff Resources.
  2. Review communications from camp. Camps emailed summer trip leaders on February 3 with guidance about the new health form for nationally-managed camp trips. For camp-specific questions and requirements, reach out to the camp.
  3. Answers to your questions. As you are learning, if you have questions about using the new health form in YL Connect, or if parents have questions, reach out to the Connections Team.  If you have questions about what constitutes as an additional activity, reach out to Risk Management.

If you have a trip less than 72 hours, or a high-risk event before May 20, you can continue using the former forms and process! Beginning May 20, 2026, all U.S. trips and high-risk events (regardless of length or location) will use this new health form process.
