We’re currently experiencing a submission issue with the online 6007 Health and Consent form due to a system update made on June 19. Users who started their Health and Consent form but did not submit the form before the system update are now encountering a submission error. The issue stems from the fact that their in-progress forms don’t reflect the latest system version.
What to Do
- Anyone who started an online Health and Consent form but did not submit it by June 19 should create a new form. When a new form is created, the user does not receive a submission error.
- As of June 20, we’ve fixed an issue that was preventing adult guest types from submitting a new form. All guest types can resubmit a new Health and Consent form.
- Users should exit out of the form to create a new form and not just refresh the page or navigate back to page 1.
- Anyone starting a brand new Health and Consent form will be able to successfully submit.
